The web-GEMs library continues to grow with the addition of two new web-GEMs, focusing on hazards of hospitalization.

Hazards of Hospitalization: Transitions of Care and Discharge Planning

You are on the hospital inpatient service and are taking care of Patty Smith, a 70 year-old woman admitted three weeks ago with shortness of breath and lower extremity edema. This is her third admission to the hospital in the past six months. She has a history of congestive heart failure (CHF), hypertension, coronary artery disease and osteoarthritis. She lives at home with her husband. She is now medically stable and your team is getting ready to discharge her.

Hazards of Hospitalization: Patient Safety in the Acute Care Setting

Mr. Mikesh is a 74 year-old man, who presented recently to his primary care physician with a 6 month history of weight loss, bloating and nausea, constipation, and bright red blood per rectum. He was sent for a colonoscopy and CT scan of the abdomen and pelvis that found a left sided colon mass. Biopsy showed adenocarcinoma so he was referred and electively admitted to the colorectal surgery service for a left hemicolectomy.
